The "paper" WOrk
UCPS will be using the form designed by the NCHSAA to pre-screen all students returning to sports and marching band. The form is shown below and all answers will be captured electronically.
In addition, UCPS is asking all students and adults returning to activities during the summer of 2020 to sign a Waiver/Release for Communicable Diseases including COVID-19. This form is captured electronically below.
In addition to the forms above, if you have not RSVP’d for band camp yet please do so and also filling out the sizing information we are still ordering "show shirts" as well as freshman items! There are three separate forms that all MUST be completed
(Band camp survey and sizing need to be done even if you or your child are not attending band camp)

Safety Precautions
Safety is important to us! We have a strict set of guidelines created by UCPS that must be followed in order to insure the health of our band members.
- All Students will be required to fill out an initial screening form before returning or entering rehearsal for the 1st time.
- Daily Screenings before entering rehearsal for the first time.
- Daily temperature checks before entering rehearsal
- Rehearsal groups are limited to 25 persons (Including instructors) on a single practice field and can last no longer than 90 minutes.
- All PRHS band students are required to maintain a social distance of 15 feet in order to mitigate the dangers of airborne water droplets or aerosols.
- After the first rehearsal students must take all instruments home and bring them each day so as to not enter the building or the enclosed storage space.
- No sharing of instruments, mouthpieces, or sticks.
- Students must bring their own water and hydration system to practice.
- All persons must wear a mask when not playing at all times including students and instructors/directors.
Band Camp Volunteer Needs
Volunteering at band camp will be extremely limited this year due to safety restrictions. If you would like to volunteer at band camp, please see the options below. Please make sure you registration with UCPS is also current. No current registration with the county means no volunteering
